Chocolates, although those might send a certain kind of message, depending on the person. (Flowers also fall under this category) A nice dinner out. A good book.

Oh wow. I wish my friend Pinkie Pie was here. She's pretty much the queen of birthdays. She had this thing called a party cannon---she pulled the string and cake, balloons, and streamers would come shooting out and perfectly put themselves in place.

Knowing as little as I do about the person and the gift, most of this will be up to your discretion, but it sounds as if you might have a good place to start. If that won't work, perhaps a gift of food or some kind, if you weren't already considering that? I haven't known many to turn down a favorite treat or a rare fruit they like.
